To further protect themselves and their families, full-time and management associates can purchase additional Optional Term Life Insurance, Dependent Term Life for spouse/partners and/or child(ren), and Accidental Death & Dismemberment (AD&D) Insurance issued by Prudential.

Why do I need life insurance?

Life is full of pleasant surprises and, at the same time, life holds many uncertainties. It’s easier to plan for happy events you know will occur, such as buying a home, paying for a wedding, or saving for college tuition costs. It’s more difficult to plan for the unexpected—such as a death.

For these times, it’s important that you have enough life insurance coverage for you and your family. Unfortunately, like many people, your current life insurance protection may be inadequate. You may be underinsured if your salary has increased since you last purchased insurance. Plus, when you consider new family responsibilities and inflation, the life insurance coverage you have now may not offer enough protection for your family.

What coverages are available to associates?

Company-Paid Insurance

ASSOCIATE ONLY

Basic Life Insurance (Management and full-time associates)

  • Once you have become benefits eligible, you are automatically enrolled for one times your covered annual earnings, rounded to the nearest $1,000, not to exceed $50,000.
  • The accelerated benefit option allows for an early payment of 50% of your coverage amount if you are terminally ill with a life expectancy of twelve months or less. Your death benefit will be reduced by the accelerated death benefit amount you receive.*
  • Coverage will end on your termination of employment. In most cases, you can continue all or a portion of your coverage. To request information on all available continuation options, contact 877-740-2116.

Associate-Paid Insurance Options

ASSOCIATE ONLY

Optional Life Insurance

  • Coverage is available for $25,000, $50,000, $75,000, $100,000, $150,000 or $200,000.
  • Management associates can purchase $300,000, $500,000, $750,000, or $1,000,000.
  • The accelerated benefit option allows for an early payment of 50% of your coverage amount if you are terminally ill with a life expectancy of twelve months or less. Your death benefit will be reduced by the accelerated death benefit amount you receive.*
  • Coverage will end on your termination of employment. In most cases, you can continue all or a portion of your coverage. To request information on all available continuation options, contact 877-740-2116.

Newly Eligible Associates: You can select $25,000 without providing proof of good health, if you apply when first eligible for benefits at Walmart.

Late Entrants: Proof of good health is required for all coverage amounts if you are enrolling in coverage after your newly eligible status ends.

*Important Notice: The acceleration of life insurance benefits offered under this certificate are intended to qualify for favorable tax treatment under the Internal Revenue Code of 1986, IRC Section code 101(g). If the acceleration of life insurance benefits qualify for such favorable treatment, the benefits will be excludable from your income and not subject to federal taxation. Tax laws relating to acceleration of life benefits are complex. You are advised to consult with a qualified tax advisor about circumstances under which you could receive acceleration of life insurance benefits that are excludable from income under federal law.

SPOUSE/PARTNER

Optional Dependent Life Insurance - Spouse/Partner

  • Coverage is available for $5,000, $15,000, $25,000, $50,000, $75,000 or $100,000.
  • For a spouse/partner confined for medical care or treatment at home or elsewhere, coverage will begin when confinement ends.
  • Coverage will end on your termination of employment. In most cases, you can continue all or a portion of your coverage. To request information on all available continuation options, contact 877-740-2116.

New Hires: You may select to enroll your spouse/partner for $5,000, without providing proof of good health, if you enroll your spouse/partner when first eligible.

Late Entrants: Proof of good health is required for all coverage amounts.

CHILDREN

Optional Dependent Life Insurance - Child(ren)

  • Coverage is available for all your children for $2,000, $5,000 or $10,000.
  • No proof of good health is required.
  • Coverage begins at live birth and continues to age 26.
  • For dependent children confined for medical care or treatment at home or elsewhere, coverage will begin when confinement ends.

Note: If both spouse/partners are benefit-eligible Walmart associates, double coverage is allowed for both spouse/partner and children.

How much does Optional Life Insurance cost?

Rates may change as the insured enters a higher age category, also rates may change if plan experience requires a change for all insureds.

How much does Dependent Life Insurance cost?

SPOUSE/PARTNER

Rates are based on your age (not your spouse/partner's age) and whether or not your spouse/partner is tobacco free. You can find a complete list of rates on the WIRE and www.walmartone.com in the Associate Benefits Toolkit.

CHILD(REN)

The cost of dependent life insurance for your child(ren) is based on the coverage amount you select and a flat rate, regardless of the number of children covered. The chart below shows the cost per bi-weekly pay period.*

How do I enroll for Optional and/or Dependent Life Insurance?

You can enroll for or increase your optional and/or dependent term life insurance any time of the year by visiting the WIRE or www.walmartone.com. However, if you enroll for or increase your amount after you are first eligible, you will be required to provide proof of good health for yourself and/or your spouse/partner.

How do I check the status of my Proof of Good Health application?

If you have completed a Proof of Good Health application, you can check the status of that application by visiting https://medu.prudential.com/meduemp. The Walmart control number is 43939.

How do I change my beneficiary?

You can change your beneficiary on the WIRE or by visiting www.walmartone.com.

How do I continue coverage for my dependents or myself, if my group coverage ends?

If you or your dependents are no longer eligible to receive life insurance benefits through Walmart Stores, Inc., you may be eligible to continue some or all of your coverage depending on the terms of the Booklet-Certificate. You must request the continuation coverage and pay the first premium within 31 days of the date your group coverage ends.
